This is a classic continuity error. They have used a shot of the male actor reacting to what Phoebe says, but Phoebe herself is in the shot.
Maybe this is where they got the idea for Phoebe’s twin from. There’s two of her in the pilot, so maybe she has a twin. And Ursula was born. It’s a nice idea, but clearly wishful thinking.
